Our Enrollment Packs are available in the office - or you can download the enrollment form, ensuring that ALL of the pages are signed and completed. We will require the original at your earliest convenience, if you email the completed forms to us. If you require a quote, you will need your CCB % to enable us to give you an accurate 'out of pocket' expenses quote. We CANNOT accept any child until the relevant forms are filled in, signed and handed in.

Extra Curricular Activities Permission

Kids Club supply afternoon tea for After School Care and a nutritious lunch for Vacation Care. The menu and program is generally posted on the front white board at the entrance each day. Also a menu for Vacation Care will be placed with the program for the vacation period.

Kids Club runs a courtesy bus to ALL of the schools in Dalby and if your child requires transport to a sport/dance after school, we will drop them off to the location of that activity. This service will only be offered if it can be incorporated within the normal bus run of the afternoon.

Homework supervision is offered. Please see staff if you would like the staff to supervise your childs homework. Please note, staff cannot sign it off as completed - Parents must do that.
